Print, fill out, and turn in completed Daily Bulletin Forms to Room 24 by 1:30 PM the School Day before the Announcement Date —AT THE LATEST. These Forms can also be found in the Student Center and Room 24. The Office is in charge of creating the Daily Bulletin. This Form is for Leadership, Clubs, and Sports to use.
Fundraiser or Activity Request Forms must be completed before any Fundraiser or Activity On-Campus or Off-Campus occurs. Completed Forms can be turned in to the "BROWN FORM" Cubby in the Student Center. Please file any Fundraiser or Activity Forms at least two weeks prior to the fundraiser/activity. Forms are usually passed in weekly Business Meetings. You will be contacted once your Fundraiser or Activity is approved or rejected. If you have any questions, please contact email@example.com. HOW TO FILL OUT A GUEST PASS:
1. The WHS Student and Guardian must fill out the Section labeled "Step One."
2. The Guest, the Guest's Guardian, and an Administrator from the Guest's High School must fill out the Section labeled "Step Two." If the Guest is not a High School Student, they must attach a Valid ID to the Guest Pass instead.
3. Once the first two Sections are filled out, turn the Guest Pass into the Office.
4. Once the Guest Pass has been approved, pick up the Guest Pass from the Office. The approved Guest Pass must be shown when buying a Dance Ticket for the Guest in the Student Center.
5. Hold on to the Guest Pass and turn in the Guest Pass to the Check-In Table at the Dance.
**Guest Passes are usually due to the Office by the Wednesday before a Dance. You must give the Office ample time to process your Guest Pass. All Dances allow Guests with the exception of the Hello Dance. These Forms can also be found in the Student Center.
Print, fill out, and turn in Publicity Request Forms to the "Completed PR Forms" Folder that is taped to the PR Door in the Student Center. Submit AT LEAST three School Days before you would like your Request to be made, or else it will not be made in time. These Forms can also be found in the Student Center. This Form is for Leadership, Clubs, and Sports to use. If you are an Individual who would like a Dance Ask Poster, you must pay for it at the Student Store before submitting your Request Form.
To request Transcripts, please turn the Forms in to the Registrar's Officer in the Office. For current Students, the first Transcript Request is Free and then $2 per Transcript for all Requests after. Transcripts requested for Scholarships are FREE. Please allow five Working Days for the Registrar to complete Transcript Requests. These Forms can also be found in the Student Center.
Work Permits are required of anyone who wants to work that is under 18 Years and is still in High School. Once you have graduated from High School, you are no longer required to have a Work Permit. Completed Work Permit Forms can be turned in to the College/Career Center (located in the Student Center). You MUST bring your Original Social Security Card to have a Work Permit processed. These Forms can also be found in the Student Center.
Club registration forms can be picked up inside the student center and can be turned in to the “Completed Club Registration Forms” box. They will be discussed during business meetings on Thursdays. The email written on the registration form will be notified as to whether the club was passed or not. Clubs may not have an adviser that already is advising over three clubs. Poster request forms are available to publicize events, announcements, etc. Posters will be made by the PR Leadership commission. Previous poster request forms were located in the student center PR Room.
