Fundraiser or Activity Request Forms must be completed before any Fundraiser or Activity On-Campus or Off-Campus occurs. Completed Forms can be turned in to the Black Box next to the door in the Student Center.

Please file any Fundraiser or Activity Forms at least TWO WEEKS prior to the fundraiser/activity. Forms are usually passed in weekly Business Meetings. You will be contacted once your Fundraiser or Activity is approved or rejected. If you have any questions, please contact ioc@washingtonasb.com.

service learning form

All service hours are now submitted online through your specific Service Hour Google Classroom (check the Service Hour page for codes). Service hour forms may be picked up at the Student Center or printed through the link above. Make sure to read requirements for the approved programs on the school website.

Club registration form
